1993年以来,被美国食品与药品管理局(FDA)认可的有关CKD进程的临床研究替代性诊断终点指标为“血清肌酐浓度为基线值的两倍”,其相当于估算肾小球滤过率(eGFR)降低57%。而以两倍肌酐浓度或终末期肾病(ESRD)为诊断终点的临床研究一般需要随访五年左右的时间。 

随着eGFR评估方法的改进,使得采用更低血清肌酐浓度用以替代两倍血清肌酐浓度作为CKD研究诊断终点指标具有了可行性。采用“eGFR少量下降”作为CKD进程替代诊断终点,可以缩短随访时间、降低研究费用,并增加临床研究效率。 

因此,慢性肾脏病预后联盟(CKD-PC)的专家团队针对“eGFR少量下降”作为CKD进程替代诊断终点的临床研究价值进行了这项荟萃分析。作者通过分析eGFR三年的变化与ESRD和死亡率之间的关系,分析了eGFR变化率在不同肾功能基线亚群中的诊断效能和一致性,为评估潜在的可替代性CKD诊断终点提供证据支持。 

在这篇国际化的荟萃分析中,作者根据CKD-PC的标准选择研究项目,目前CKD-PC由50项研究组成,提供血清肌酐浓度、尿蛋白结果及超过50项的预后数据。

其中有22项研究(4个普通人群,5个高心血管风险,13个CKD相关研究)具有6个月到3.5年反复测量的血清肌酐浓度数据,可以确定eGFR变化率与ESRD预后的关系。有35项研究(15个普通人群,7个高心血管风险,13个CKD相关研究)具有死亡结果数据。 

本研究汇集了170万被调查者,包含12344例ESRD病例和223944例死亡病例。通过分析,作者证明小于两倍血清肌酐浓度的eGFR降低,与ESRD风险具有显著和一致的相关性,并且可以获得更高比例的ESRD风险,为“eGFR少量下降”做为CKD进程的替代诊断终点提供了数据基础。 

分析显示,eGFR变化越大,经首次eGFR和其他变量校正后的ESRD风险比(HR)越高。eGFR变化57%,HR增高30倍,eGFR变化30%,HR增高5倍,但是后者的发生率比前者高10倍,所以具有较高的人群分布风险(PAR分别为44%和10%)。

死亡风险显示出了与ESRD风险一样的相关性特征。这些数据为我们理解高风险与低发生率之间的平衡提供了依据。当我们进行CKD进程研究时,选择一个较高或较低eGFR百分比变化作为诊断终点,尽管HR的趋势在不同研究中是一致的,但是不同eGFR诊断终点的选择使研究中被调查者的特征具有显著差异。 

文中强调了几点:

(1) 不仅仅要进行ESRD预后结局的研究,其他预后结局(如心血管疾病和死亡)由于发生更加普遍并可以发展成ESRD,所以对其评估也是重要的。分析显示,十年间,eGFR降低30%的群组死亡率为50%,而eGFR稳定组,死亡率为32%。 

(2)评估临床研究时,必须评估治疗效果的替代终点与临床终点的一致性。而替代终点带来的益处更大。 

(3)由于CKD研究的数量和类型有限,特别是随访期限和EDRD事件数量,所以需要模拟研究来评估潜在情况,以评价eGFR少量降低作为CKD研究诊断终点的有用性、稳健性和权威性。
 
作者通过荟萃分析,以大量研究数据为基础,证明“eGFR少量降低”比“两倍血清肌酐浓度”的存在更加普遍,与ESRD和死亡风险之间的关系更加显著和一致。建议考虑以“eGFR少量降低”(例如,2年间eGFR降低30%)作为替代性的CKD进程的诊断终点。
