Prostate Cancer P D：前列腺癌患者中，家族癌症历史对第二癌症风险和死亡率的影响

生存率在前列腺癌患者中不断增加，且在这些患者中第二原发癌症（SPCs）越来越普遍。然而，SPCs的病因和临床结果并没有很好的阐释。最近，有研究人员确定了家族历史对这些患者的SPC和死亡原因的影响。研究发现，SPC在6396名男性中诊断，并且超过1/3的患者具有癌症的以及家族历史；具有家族历史的风险为1.37（95% CI： 1.27-1.40），没有家族历史的为1.10（1.08-1.16）。前列

生存率在前列腺癌患者中不断增加，且在这些患者中第二原发癌症（SPCs）越来越普遍。然而，SPCs的病因和临床结果并没有很好的阐释。最近，有研究人员确定了家族历史对这些患者的SPC和死亡原因的影响。

研究发现，SPC在6396名男性中诊断，并且超过1/3的患者具有癌症的以及家族历史；具有家族历史的风险为1.37（95% CI： 1.27-1.40），没有家族历史的为1.10（1.08-1.16）。前列腺癌在年龄到83岁时累积的发病率达到了21%。在这些患者中，SPC占28%；而那些具有SPC和家族历史的占比35%。另外，家族历史与7中特异性的SPCs相关，包括结直肠、肺、肾、膀胱及皮肤(黑色素瘤及鳞状细胞)癌及白血病，并且家族历史能够成倍增加这些SPCs的风险。在患有SPC的患者中，死亡原因的一半是SPC引起，只有12.77%的死因归结于前列腺癌。另外，SPC中的大多数死亡是由肺癌和大肠癌引起。

最后，研究人员指出，SPCs是前列腺癌患者死亡的重要原因，并且家族历史是SPCs的重要风险因子。在前列腺癌生存率增加的同时，SPC的预防是必要的，可以通过前列腺癌诊断时对家族历史进行评估来着手进行。
