85岁男性，表现为反复呕吐和眩晕48小时，伴快速进展性吞咽困难和四肢瘫痪。

85岁男性，表现为反复呕吐和眩晕48小时，伴快速进展性吞咽困难和四肢瘫痪。MRI-MRA可见延髓旁正中梗死（图A-C），称“心形”征，这是一种罕见的后循环卒中特点，因脊髓前动脉闭塞所致（图D）。临床表现由其前后受累部位决定。四肢瘫痪，舌肌无力和感觉缺失，分别定位于锥体束，舌下神经核和内侧丘系，本例有反复呕吐，双侧面部瘫痪和水平性眼肌麻痹，提示病灶延伸至脑桥，伴极后区，面神经丘和旁正中脑桥网状结构受累。
