NB水产生于NB发生器中。检测NB大小、浓度和pH值。采用猪牙手动预备至60号,并分别使用NB水, 17% EDTA冲洗或不进行冲洗。通过SEM观察冲洗剂去除玷污层的效果(9 roots/group)。其他样本(6 roots/group)进行Vickers硬度测试以确定牙本质的显微硬度。将自发荧光四环素分别与去离子水或NB水混合并置于猪牙根管内,通过体式荧光显微镜观察药剂渗入牙本质小管内的深度(5 roots/group)。对于去感染实验,预备牙根、高压灭菌,粪肠球菌孵育3周。随后分别通过以下环节实施根管去感染:(1) 5.25% NaOCl标准针头冲洗(SNI), (2) 5.25% NaOCl通过超声活化(US), (3) 5.25% NaOCl + XP finisher (XP), (4) SNI和1.5% NaOCl, 或(5) 含SNI和1.5% NaOCl的NB水; (5 roots/group)。将牙齿劈开,通过LIVE/DEAD BackLight染色,使用激光共聚焦显微镜(CLSM)观察根管的冠、中和尖1/3。计算牙本质小管内不同深度水平(50 μm, 100 μm, and 150 μm)的死/总细菌比例。

结果显示,NB水去除玷污层的效果明显优于17% EDTA,并且能够促使四环素药物渗透入牙本质小管超过1 mm的深度。与17% EDTA相比,NB水并未改变牙本质的显微硬度(P<0.05)。在50 μm的深度,CLSM结果显示,在根管冠、中和尖的区域含1.5% NaOCl的NB水和5.25% NaOCl在有或无冲洗活化的情况下均无明显差异(P>0.05),比如:这些组比1.5% NaOCl具有更强的杀菌效果(P<0.05)。在更深的水平(100 μm and 150 μm),高浓度的NaOCl的效果比1.5% NaOCl更高,无论有或无NB水。在大多数深度水平,5.25% NaOCl不论是有或无活化其效果均无显著性差异(P>0.05)。

结论:NB水能够去除玷污层并增强药剂在牙本质小管内的渗透,同时不改变牙本质的显微硬度。在较大的根管模型中,NB水能够提高低浓度NaOCl在牙本质小管内的去感染能力,深度可达到50 μm。另一方面,冲洗活化(US or XP)与标准的针头冲洗相比并不能提供任何附加的牙本质小管内的去感染效果。这些结果表明,NB水可能是牙髓冲洗和封药的一种理想附加物。
