Crit Care:严重革兰氏阴性感染后单核细胞产生的细胞因子减少!

循环中单核细胞不能产生足够多的细胞因子是脓毒症引起免疫抑制的特点,但是,医务人员对其持续时间和最终结局了解甚少。近日,危重病医学领域权威杂志Critical Care上针对这一问题发表了一篇研究文章。

研究人员进行了一项大规模的随机临床试验研究。研究人员从95例病原学证实或临床怀疑革兰氏阴性菌感染的患者全身炎症反应综合征发生的头24小时内分离出外周血单核细胞(PBMC)。此外,在发病后第3天、第7天和第10天进行重复分离。PBMCs被刺激分泌细胞因子。该研究终点是幸存者和非幸存者之间的差异,持久的免疫抑制,以及确定的入院临床症状,这些症状可以导致早期识别免疫抑制的可能性。

3天后幸存者的PBMCs产生较大浓度的肿瘤坏死因子α(TNF-α)、白细胞介素(IL)-6、IL-8、IL-10、干扰素γ和粒细胞巨噬细胞集落刺激因子。使用ROC曲线分析,研究人员发现在第3天内毒素刺激后TNF-α生产小于250pg/ml,可以区分患者与健康对照者;这种相关性有5.18的比值比,会出现一个不利的预后(P=0.046)。这个特点持续到第10天。Logistic回归分析显示入院时血管衰竭是第3天TNF-α产生下降的唯一独立预测因子。

由此可见,TNF-α的产生下降是脓毒症免疫抑制的主要特征。它与不利结果的显著性风险相关,并且持续到第10天。在随访期间,入院时血管衰竭可以预测TNF-α的产生下降。
