BMJ:肺栓塞诊断预测:Wells模型性能更佳

荷兰一项研究表明,5种易在初级保健机构中应用的肺栓塞诊断预测模型效率相当,但韦尔斯(Wells)模型因失败率较低而性能表现最佳。论文9月8日在线发表于《英国医学杂志》(BMJ)。这项结果与此前另一项研究结果相似:BMJ:Wells评分结合D-二聚体检测有效排除深静脉血栓  该研究先通过系统评价后再进行独立外部验证,对易于在初级保健机构应用的所有排除肺栓塞诊断预测模型进行评估。所

该研究先通过系统评价后再进行独立外部验证,对易于在初级保健机构应用的所有排除肺栓塞诊断预测模型进行评估。所有模型均通过系统文献检索得到,并通过计算和比较C统计值来评估其鉴别能力。依据联合定性D-二聚体检测的预设模型阈值,将598例疑似急性肺栓塞患者分层为肺栓塞高和低概率组,并计算所有模型的敏感性、特异性、效率(肺栓塞低概率患者的总体比例)和失败率(肺栓塞低概率患者组中肺栓塞病例比例)。

结果显示,共发现用于肺栓塞诊断的10种预测模型,其中5种模型可在初级保健机构数据集中加以验证,分别为原始Wells、改良Wells、简化Wells、修订Geneva和简化修订Geneva模型。所有模型的鉴别能力相当。敏感性介于88%(简化修订Geneva)至96%(简化Wells)之间,特异性介于48%(修订Geneva)至53%(简化修订Geneva)之间。所有模型的效率为43%~48%。失败率之间存在差异,在简化Wells和简化修订Geneva模型之间尤为显著(失败率分别1.2%和3.1%)。不考虑所用诊断预测模型时,有3例患者被错误分类为肺栓塞低概率;肺栓塞只有在转诊至二级医疗机构后才得以诊断
