住院病历

主诉：发现踏上医学贼路后悔8年，感觉进行性加重3年

现病史：患者于2005年高考后在家人及朋友的坑蒙拐骗下出现阵发性对从医事业的诡异想，同时伴有对美好未来的憧憬、对理想执着追求等异常亢奋心理表现。自2005年报考医学院饱受解剖学，生理学，病理、免疫、病生，内外妇儿等摧残后出现营养不良，目光呆滞，抑郁寡欢，伴自残倾向。症状时轻时重，未加重视且未进行规范诊疗。2010年在医院实习，兼任护送工人、专业打饭户、顶气包、清洁工等职务后上述症状逐渐加重，伴有对未来生活的彻底失望，对忙碌工作的厌恶心理，今为求治疗来我院就诊，门诊以“21世纪傻逼学医综合征 III级极高危”收入我科。

发病以来患者精神恍惚，常因没时间摄食狼吞虎咽，失眠，大小便后怕忙碌不愿出厕所，体重明显减轻（具体不详）。

既往史：5 岁时脑子进水2两，未正规治疗；2005年脑子生虫，无钱医治。

个人史：无烟酒等奢侈嗜好；常居昏暗潮湿集体房，处刀光剑影流窜住院部，未见阳光及不跑腿不太适应；有熬夜“看书”、“考试”、“发文章”等不良嗜好。

过敏史：对“夜班、急诊班”过敏，对“老师及主任问问题”过敏；尤其对“半夜护士敲门及电话声”超级敏感。

婚育史：未婚未育甚至无对象。

家族史：后代中从此再不会出现医生。

体格检查 T: 36.4℃， P：100次/分， R: 20次/分，BP：178/94mmHg。发育尚可，营养不良，神志恍惚，目光呆滞，苦笑面容。遇病人时“强迫正经体位”，遇上级医生时“强迫紧张体位”，遇领导时“被动谦虚体位”，遇同学时“自动猥琐体位”。头发蓬乱，面色苍白，双眼挂满黑眼圈，颈部常挂有听诊器，身穿带有莫名其妙墨迹的白大褂，口袋有翻破的笔记本、蓝黑色笔、红色笔及乱七八糟小卡片、口罩袋、小手电筒、棉签纱布甚至眼药水等杂物。

辅助检查： 2013年我院门诊脑电图示：大脑皮层出现异常形状不规则、凌乱的“﹠﹡&﹎ & %”傻逼波。

头颅MR示侧脑室内可见无数大小不等、形状各异的毛毛虫，脑皮质萎缩，中线结构扭曲。

入院诊断：

1. 21世纪傻逼学医综合征 III级 很高危

2.脑进水

3.脑残
