我国心血管病发病率及病死率呈逐步上升趋势,已经成为威胁人群健康和生命的严重公共卫生问题。建立科学合理的心血管事件的预测体系,有利于早期识别、预防心血管事件。传统的心血管危险因素,如血脂、血压等是个体心血管事件风险评估的重要指标。近期国际临床检验推出更敏感更特异的方法检测心肌肌钙蛋白(cardiac troponin,cTn)的方法。研究表明,高敏肌钙蛋白( hs-cTn

我国心血管病发病率及病死率呈逐步上升趋势,已经成为威胁人群健康和生命的严重公共卫生问题。建立科学合理的心血管事件的预测体系,有利于早期识别、预防心血管事件。传统的心血管危险因素,如血脂、血压等是个体心血管事件风险评估的重要指标。近期国际临床检验推出更敏感更特异的方法检测心肌肌钙蛋白(cardiac troponin,cTn)的方法。研究表明,高敏肌钙蛋白( hs-cTn)不仅实现了对急性心肌梗死的极早期诊断,而且对于急性冠状动脉综合征、心力衰竭、稳定性冠心病患者,甚至是一般人群的心血管事件均具有重要的预测价值。

一、hs-cTn的概念

cTn是由cTnT、cTnI、cTnC 3个亚单位组成的球形分子,是心肌收缩单位的重要调节蛋白。在心肌细胞内,绝大部分cTn以复合体的形式结合在细肌丝上,少量的cTn以游离形式存在于胞浆中,正常情况下健康个体外周血中检测不到cTrl.当心肌细胞受损时,细胞膜通透性增高,游离状态的cTn首先释放到邻近的心肌间质、淋巴管和微血管中,随血液循环逐渐进入外周血中;随着心肌细胞损伤加重产生持续肌丝崩解,大量的cTn持续释放至血液中。高浓度cTn会持续7d以上,峰值多出现在严重缺血后1-2 d,峰浓度可达到正常上限的20-50倍。因此,正是由于肌钙蛋白这种非“全或无”的释放方式,使得我们研究极低浓度水平的肌钙蛋白与心血管疾病的相关性具有重要的意义,这种极低浓度水平的肌钙蛋白可能用来预测心血管疾病的风险。

由于编码肌钙蛋白T、肌钙蛋白I的基因在骨骼肌和心肌中不相同,只有cTnT、cTnI是心肌损伤的特异性标志物。临床检验中传统应用的是第4代eTnT检测试剂,它能够检测的低限值是0.01μg/L,10%变异系数为0.035μg/L.最近,国际临床检验学出现了用更加敏感、特异的方法检测肌钙蛋白即hs-cTn.hs-cTn不是一个新的心脏生物学标志物,而是灵敏度更高,能够检测到低于传统肌钙蛋白低限值10倍的水平。其中,hs-cTnT检测下限为0.003μg/L,正常人群99百分位数为0.014μg/L.因此,hs-cTn符合了理想心脏损伤标志物的要求。

二、hs-cTnI对心血管事件风险的预测价值

hs-cTnI的出现除了能够早期发现和诊断急性心肌梗死,还对于心血管事件具有预测价值。Tsutamoto等随访258例稳定性心力衰竭患者,中位随访时间为2.6年,20例因心脏事件死亡,hs-cTnI对于心脏事件死亡的检测率是43%,而cTnT的检出率仅为12%.Kavsak等评价了HOPE研究中2572例高心血管风险人群的心肌梗死、卒中、心血管死亡等终点事件,平均随访4.6年,结果表明,hs-cTnI为心肌梗死、心血管死亡提供了良好的预测信息,但对于卒中没有预测价值。最近的研究也表明,动态监测hs-cTnI水平与基线hs-cTnI水平相比对心力衰竭死亡具有更大的预测价值。

三、hs-cTnT对心血管事件风险的预测价值

由于hs-cTnT的出现提高了普通人群cTnT的可检测率,国际上开始探索性研究hs-cTnT作为心脏生物标志物对人群未来心血管事件的预测价值。deFilippi等报道了一个纳入4221例社区老年居民(65岁以上,无心力衰竭)、随访中位时间11.8年的全国纵向队列研究。结果显示,hs-cTnT>0.013pg/L者,每100例心力衰竭、心血管死亡明显高于hs-cTnT<0.003μg/L者。hs-cTnT基线水平高于0.003μg/L者随访期升高大于50%组其心力衰竭、心血管死亡的风险显著增高。

de Lemos等在一个纳入3546名普通参与者(多民族,30-65岁)、平均6.4年的队列研究中发现,hs-cTnT(≥0.003μg/L)的可检测率约为25.0%,其中男性、女性中的比例分别为37. 1%和12.9%,40岁以下和大于60岁者的比例分别为14.0 %和57.6%.随访期间的全因死亡率随hs-cTnT水平而增高。调整传统心血管危险因素、C反应蛋白水平、慢性肾病、NT-proBNP水平等因素后,hs-cTnT水平是全因死亡的独立危险因素。

Saunders等在ARIC研究(纳入9698例基线时无冠状动脉心血管疾病、卒中,年龄54-74岁的普通参与者)中发现,可检测到hs-cTnT(≥0.003μg/L)者占66.5%,hs-cTnT>0.014μg/L者占7.4%.调整传统心血管危险因素、肾功能、高敏C反应蛋白水平、NT-proBNP水平后,冠心病、死亡、心力衰竭住院的发生率等随hs-cTnT水平的增加而增高。hs-cTnT水平最高组(≥0.014LLg/L)的冠心病、致死性冠心病、总死亡和心力衰竭的风险均明显增高。即使是hs-cTnT中间组。

Everett等分析了女性健康研究中糖尿病与非糖尿病参与者基线cTnT与心血管事件(心肌梗死、卒中、心血管死亡)的关系。hs-cTnT在糖尿病女性的检出率高于非糖尿病女性(45.5%:30.3%)。调整了传统的心血管危险因素及糖化血红蛋白、NT-proBNP和估算的肾小球滤过率(eGFR)后,糖尿病女性的hs-cTnT与心血管事件独立相关。因此,hs-cTnT对糖尿病女性未来心血管事件的发生可能具有预测价值。

四、hs-cTn在国人中的研究进展

上述研究结果均来自于大规模的临床及流行病学研究,国内相关研究不多,相对滞后,有必要探索hs-cTn在中国人中的应用价值。我们的研究团队于2007年9月至2009年1月建立起北京部分地区心血管病危险因素数据库,入选了4895例社区常住成人居民,包括一般情况、体格检查、生化指标、新的心脏生物学标志物、DNA样本库以及超声心动图、动脉僵硬度等指标。其中在北京苹果园社区,按照入选排除标准纳入1601例年龄在45岁以上居民进行健康体检,1503例提供血标本检测hs-cTnT,最终完成所有项目检测、资料齐全的1497例进入队列研究,这也是我国第1个有关一般人群hs-cTnT分布、相关因素以及其对心血管病风险预测的研究。

我们的研究显示,1497例社区中老年人群中816例(54.5%)可检测到hs-cTnT(>0.003μg/L),178例(11.9%)hs-cTnT高于正常参考值(≥0.014μg/L),合并高血压、糖尿病、冠心病及肾功能下降人群hs-cTnT的检测率升高。排除132例明确诊断心血管病的患者(确诊的冠心病和缺血性卒中)后,以0.003μg/L为检测点,将人群分为hs-cTnT可测与不可测组,结果显示,可测组的年龄、男性、吸烟史、高血压病史、糖尿病病史、收缩压、脉压、心率、总胆固醇、空腹血糖、NT-proBNP均高于不可测组,而高密度脂蛋白胆固醇( HDL-C)和估算的肾小球滤过率(eCFR)水平低于不可测组。多元逐步回归的结果表明,男性、年龄、空腹血糖、eGFR、NT-proBNP与hs-cTnT独立相关。

我们进一步分析了hs-cTnT与预测的心血管病风险的相关性。以经典的Framingham冠心病风险评分和心血管病风险评分来预测心血管病风险,结果显示,高风险组hs-cTnT水平高于中等和低风险组(包括Framingham冠心病风险和心血管病风险),调整其他混杂因素后,随着hs-cTnT水平递增,Framingham冠心病风险及心血管病风险评分均增高。但另一方面,无论是单因素还是多因素分析均显示,hs-cTnT水平递增性上升与经典的Framingham冠心病风险评分和心血管病风险评分并不是简单的直线关系。

无论是Framingham冠心病风险评分还是心血管病风险评分仅涵盖了传统心血管危险因素的影响,而未涉及新型或非传统的心血管危险因素。hs-cTnT水平与Framingham评分和心血管评分的相关性,在本质上是hs-cTnT与传统危险因素的相关性。进一步分析发现,除了诸如男性、年龄等传统心血管危险因素与hs-cTnT水平相关外,NT-proBNP水平、GFR、左心室肥厚及动脉硬化指标也与hs-cTnT水平独立相关。鉴于hs-cTnT水平与心血管死亡及总死亡的密切关系,提示我们检测hs-cTnT水平可能作为一项有益的心脏生物学标志物,有助早期发现心血管病高风险人群。

动脉硬化是心血管事件的独立危险因素,脉搏波传导速度(PWV)是目前评价动脉硬化的“金指标”。我们探讨了社区人群颈股PWV(cf-PWV)与hs-cTnT的相关性。结果显示,在年龄60岁以上的老年一般人群中cf-PWV与hs-cTnT相关,提示大动脉僵硬度增加可能亚临床心肌损伤的原因之一,大动脉硬化与心肌损伤之间的相关性可能是其导致心血管事件增加的重要机制。

心肾损害间的相互作用一直是关注的热点。估算的eGFR下降是心血管死亡的独立危险因素,终末期肾病患者中恶性心血管事件风险明显升高,同时表现为心肌损伤标志物的持续升高。但是,在一般人群中不同程度的肾功能变化与心肌损伤的关系还不明确。为此,我们探讨了社区人群不同程度的eGFR与hs-cTnT水平的相关性,结果显示,不同四分位的eGFR水平组中,Framingham冠心病风险预测评分或心血管病预测评分均与hs-cTnT相关;在调整Framingham冠心病风险预测评分或心血管病预测评分后,仅提示中、重度eGFR下降与hs-cTnT水平相关,提示可能心血管危险因素与肾功能减退共同影响hs-cTnT水平。

业已证明,高血压左心室重构和舒张功能减退表现为一定程度的心肌损伤。为了探讨hs-cTnT水平与左心室重构和舒张功能的相关性,我们用超声心动图的左心室质量指数作为左心室重构的指标,用二尖瓣舒张早期血流速度与二尖瓣环舒张早期运动速度的比值作为左心室舒张功能的指标。结果表明,hs-cTnT水平与高血压左心室质量指数增加及左心室舒张功能减退密切相关,提示hs-cTnT水平可能有助于对无症状的高血压性左心室重构患者的危险分层和选择不同阶段的干预策略提供依据。

总之,基于中国(北京社区)45岁以上无心血管病人群的资料显示,54.5%的受试者中可测及hs-cTnT,hs-cTnT水平除了与年龄、性别、血糖等传统危险因素独立相关外,还与心血管风险积分、大血管硬化、肾功能减退以及左心室重构有关。我们的结果提示在“表面健康的”社区人群中,hs-cTnT的测及和(或)升高是传统危险因素和其他心肌致损因素的集中反映,hs-cTnT在一般人群中可能具有重要的心血管预测价值。

五、总结

综上所述,hs-cTnT对于一般人群的心血管疾病风险可能具有预测价值,有望作为新型的心血管风险预测因素,早期识别、早期干预心血管病变,为致死性心血管事件的发生提供重要的预测信息。我国目前还没有相关内容的前瞻性队列研究报告,我们的工作是我国首个在社区人群中进行的hs-cTnT对心血管疾病风险的预测研究,填补了我国相关工作的空白。2012年我们将完成研究工作的5年随访,期待着我国人群的研究数据。
