A&R:COVID-19患者中的内皮细胞活化抗体

一些COVID-19患者可能具有多种驱动内皮病变的抗体,为自身抗体在严重COVID-19中的血栓炎症作用提供了重要依据。

目的:内皮功能障碍与COVID-19的广泛血栓炎症并发症有关,静脉血栓栓塞的发生率在重症COVID-19中尤为显著(10-35%),尸检标本发现表明它影响多达60%的患者,并与COVID-19相关死亡率相关。目前内皮病的上游介质在很大程度上仍然是未知的。本研究旨在确定导致COVID-19中内皮细胞活化和功能障碍的循环因子。

方法244COVID-19住院患者的血清或血浆和100名非COVID-19相关脓毒症患者的血浆存在下培养人内皮细胞。使用细胞内酶联免疫吸附测定法对细胞粘附分子(E-选择素、血管细胞粘附分子1和细胞间粘附分子1 [ICAM-1])进行量化。

结果COVID-19患者的血清和血浆增加了细胞粘附分子的表面表达。此外,患者血清中可溶性ICAM-1E-选择素的水平升高,并与疾病严重程度相关。NETs中性粒细胞胞外陷阱/血栓炎症与COVID-19血清激活内皮细胞的能力之间存在适度相关性。循环抗磷脂抗体的存在是COVID-19血清激活内皮能力的有力标志与对照血清相比,两种COVID-19样本均显示中性粒细胞与内皮细胞的粘附显著增加。抗磷脂抗体阳性血清中总IgG的消耗显著降低了细胞粘附分子的上调。相反,用患者IgG补充对照血清足以触发内皮活化。

结论这些数据首次表明一些 COVID-19 患者可能具有多种驱动内皮病变的抗体,为自身抗体在严重 COVID-19 中的血栓炎症作用提供了重要背景。
