70岁无症状男性,行数字减影血管造影提示烟雾病(moyamoya disease ,MMD)伴基底动脉尖动脉瘤(basilar tip aneurysm,BTA)

70岁无症状男性,行数字减影血管造影提示烟雾病(moyamoya disease ,MMD)伴基底动脉尖动脉瘤(basilar tip aneurysm,BTA)(图1A-D)。与先前的研究相比,4D-flow MRI可见出高速集中喷射血流(视频,图2A-F)。考虑到血管内治疗的高风险,患者选择了保守治疗。1月后,动脉瘤破裂(图1E-F)。

因颈内动脉闭塞出现的代偿反应(图1D)可能引发血流增加,导致BTA形成和破裂。4D-flow MRI可提供准确的血流及速度,进行全面的血液动力学评估。源自MMD的高速集中喷射血流可加速动脉瘤破裂,需要迅速进行手术。
