抽烟的系统性红斑狼疮患者心梗和肾炎的风险多高？

传统的或者系统性红斑狼疮（SLE）相关的危险因素都不能完全解释SLE患者的高心血管病风险，但心血管和SLE特异的危险因素之间的相互作用鲜有研究。ARD的这篇文章则主要探究基因是如何影响抽烟相关的SLE

 探索队列的776名和重复队列的836名SLE患者均采用200K单核苷酸多态性（SNP）芯片和一个定制序列进行基因分型。分析了60个与SLE相关的SNP（p<5.0×10-8）。探索队列中，抽烟与心梗相关（OR 1.96，（95% CI 1.09-3.55）），在携带rs11889341 STAT4（信号转导和转录激活因子4）易感等位基因(OR 2.72 (95% CI 1.24-6.00))或两个易感等位基因(OR 8.27 (95% CI 1.48 -46.27))的患者中风险升高。

携带以上易感等位基因的抽烟患者患肾炎的风险也升高(OR 1.47 (95% CI 1.06- 2.03))。在重复队列中验证了携带STAT4易感等位基因与心梗(OR 6.19 (95% CI 1.29-29.79))和肾炎(OR 1.84 (95% CI 1.05-3.29))的风险升高相关。抽烟和STAT4易感等位基因之间的相互作用使心梗(OR 2.14 (95% CI 1.01-4.62))和肾炎(OR 1.53 (95% CI 1.08-2.17))的风险更高了。

心梗54%的风险和肾炎34%的风险都归因于抽烟和STAT4易感等位基因之间的相互作用。白细胞介素12（IL12）诱导的抽烟者CD8+T细胞STAT4磷酸化水平显著高于非抽烟者。IL12A rs564799 易感等位基因在探索(OR 1.53 (95%CI 1.01-2.31)和重复队列2.15 (95% CI 1.08-4.26))中均与心梗相关。

结论：   携带STAT4易感等位基因的抽烟SLE患者似乎升高心梗和肾炎的风险。该研究也强调了IL12-STAT4通路在SLE心血管发病中的作用。
