CLIN CANCER RES：使用PET辨别K19阳性肝细胞癌肿瘤干细胞

对于肝细胞癌肿瘤干细胞（HCC-CSC）的识别尚缺乏有效地工具，这一不足限制了肝细胞癌治疗的开展。有报道称keratin19（K19）是肝细胞癌肿瘤干细胞的理想标志物，18F-FDG PET是一种有效预测肝细胞癌术后结局中的手段。CLIN CANCER RES近期发表了一篇文章，研究是否可以用18F-FDG PET追踪肝细胞瘤肿瘤干细胞。

对于肝细胞癌肿瘤干细胞（HCC-CSC）的识别尚缺乏有效地工具，这一不足限制了肝细胞癌治疗的开展。有报道称keratin19（K19）是肝细胞癌肿瘤干细胞的理想标志物，18F-FDG PET是一种有效预测肝细胞癌术后结局中的手段。CLIN CANCER RES近期发表了一篇文章，研究是否可以用18F-FDG PET追踪肝细胞瘤肿瘤干细胞。

文章共纳入98例患者，在肿瘤切除前均接受了18F-FDG PET检查，使用IHC方法检测K19以及以及GLUT1的表达。根据FDG水平计算原发肿瘤部位SUV值以及肿瘤非肿瘤部位SUV比值（TNR），比较K19+与K19-患者之间的差别，检测K19+细胞与K19-细胞的18F-FDG 摄取以及GLUT1表达。作者发现在肝细胞癌患者中，K19的表达与GLUT1表达和FDG摄取显着相关。在术前的各项临床指标中，TNR是肝细胞瘤K19表达的最敏感最敏感指标。在细胞实验中，K19+细胞18F-FDG 摄取明显高于K19-细胞。进一步的实验还证明K19通过影响TGFβ/Smad通路中的Sp1及其下游的GLUT1进而调节18F-FDG的摄取。

18F-FDG PET可以用来预测肝细胞癌K19表达，对于针对K19+肝细胞癌肿瘤干细胞的治疗有一定意义。
