JAMA子刊:近八成痛风可预防,关键是控制体重!

该研究证明了生活方式干预对痛风预防的作用,尤其指出了保持体重的重要意义。

随着生活水平的提高,“大鱼大肉”成为人们日常饭桌上的常客,随之而来的高尿酸血症和痛风也成为最常见的代谢性疾病之一。其中,痛风是由高尿酸盐沉积于关节内引起的、引起关节发炎与疼痛的代谢性关节炎,发作时患者往往“痛苦不堪”,因此痛风也被认为最痛苦的病之一。

既然痛风发作如此痛苦,因此科学家们一直在寻找如何才能预防高尿酸血症甚至痛风。其中,来自哈佛医学院的专家团队最近发表在JAMA子刊JAMA Network Open杂志中的研究结果给出了新的见解。

研究人员使用了美国卫生专业人员随访研究(HPFS)中的数据,该研究共纳入51529名男性卫生人员。1986-2012年期间对该人群进行前瞻性的随访,每2年进行依次问卷调查个人身体情况。在完成随访后,共有44654名无基础痛风的参与者进行了分析。

根据汇总的问卷,将上述参与者依照下述几项因素进行分层:正常体重指数(BMI<25)、有无饮酒、是否遵循DASH饮食模式以及是否使用利尿药。

DASH饮食是一种强调蔬菜水果、坚果与乳制品均衡的饮食,提倡减少高糖食物、饮料以及红肉(如牛、羊肉)的摄入,利用高钾、高镁、高钙、膳食纤维与不饱和脂肪酸取代饱和脂肪酸以达到降血压、保护动脉硬化的作用。

在平均26年的随访后,1741名(3.9%)的参与者出现痛风发作。所有参与者中,若能做到上述四项危险因素,则能预防77%(95%CI,56-88%)的痛风发作!其中体重指数是影响最大的因素。与BMI≤23.0的人相比,BMI在23.0-24.9(正常上限)、25.0-29.9(超重)以及≥30.0(肥胖)的参与者痛风的风险分别增加29%、90%与165%。

除外体重指数,其它3个因素也分别于痛风发作的风险相关。例如,如果遵循DASH饮食模式,则能预防22%的痛风发生。而与每日摄入酒精超过30g的人相比,没有饮酒习惯的人痛风风险降低一半多。同样,不使用利尿药也能使痛风风险降低一半。

按不同风险因素组合发生痛风的相对风险。

 进一步分析发现,在保持体重(BMI<25)的同时,加上不饮酒或DASH饮食习惯,分别能预防43%与33%的痛风风险。如果能做到上述3者,则能预防69%的病例。但是,在肥胖(BMI≥30)男性中,哪怕做到这三点,痛风风险也没有降低。也就是说,保持正常体重是其它因素发挥保护作用的前提。

“这项研究对肥胖对痛风的影响进行了量化”,该研究的通讯作者,哈佛医学院麻省总院风湿免疫科Hyon K. Choi博士表示,“这提醒我们应该更加重视肥胖这一公共卫生问题,毕竟痛风会增加合并疾病和死亡风险。”

综上,该研究证明了生活方式干预对痛风预防的作用,尤其指出了保持体重的重要意义。
