尿路感染 (UTI) 是居住在疗养院 (NH) 的老年人中最常见的感染。无症状菌尿在 NHs 中也特别高，可占尿培养阳性的一半。诊断 UTI 时，临床医生通常依赖模糊、非特异性的体征和症状，例如食欲不振或尿液恶臭，这可能代表 UTI，但也存在于非 UTI 病症中。 -当 NH 注册护士 (RN) 决定就疑似尿路感染向开处方的初级保健提供者 (PCP)、医生或高级实践提供者 (APP) 打电话时，或者当该提供者决定开出不适当的抗生素时，就会开始过度诊断。虽然 RN 不进行医学诊断，但作为培训的一部分，他们会确定病情、进行评估并提供临床建议。这种培训体现在护士学校教授的情境-背景-评估-推荐 (SBAR) 技术中。因此，两种类型的 NH 临床医生对非特异性体征的误解可能会导致对 NH 居民尿路感染的过度诊断和抗生素的过度使用。多达 70% 的抗生素处方代表过度使用，并且在任何时候，10% 的 NH 居民正在接受抗生素。
