特发性肺动脉高压1例

临床资料】

患者,女,17岁。因活动后气短4年,反复晕厥2年入院。患者于4年前出现活动后气短,无胸闷、心悸、头晕等不适,未予注意。2年前出现活动后反复晕厥4次,无抽搐、尿便失禁,1分钟后自行恢复。自此活动耐量逐渐下降,上1层楼或快步行走即出现气短,口唇发绀,伴咳嗽,无咯血、胸痛。当地医院诊断为“肺动脉高压”,为进一步诊治来诊。病程中无发热、关节疼痛、口干、眼干、鼻出血等不适。

2年前曾在当地医院诊断为“焦虑症”,无减肥药接触史。未婚,月经大致正常。父母体健,无兄弟姐妹。

查体:BP 100/70mmHg,R 18次/分。双肺呼吸音清晰,未闻及干湿性啰音,双肺未闻及血管杂音。HR 74次/分,心律齐,P2亢进,胸骨左缘3~4肋间闻及3/6级收缩期吹风样杂音,双下肢无水肿。

实验室检查:血常规:RBC 4.86×1012/L,Hb 133g/L。甲状腺功能指标:正常。尿常规:正常。肝肾功能:正常。NT-proBNP:1211fmol/L。乙肝、丙肝:阴性。HIV:阴性。Hs-CRP:13.02mg/L。免疫学检查:抗双链DNA抗体、抗核抗体、抗Sm、抗RNP,抗Scl-70,抗J0-1、抗SSA、抗SSB抗体均阴性;补体C3、C4正常;IgG、IgA正常。

心电图(图10-1):窦性心律,电轴右偏,右心室肥厚,广泛ST-T改变。

X线胸片(图10-2):双肺门动脉扩张,外周纹理纤细,主动脉结不宽,肺动脉段凸出,右心房、室扩大,心胸比0.61。

超声心动图:左心房前后径31mm、左心室舒张末前后径39mm,右心室舒张末前后径31mm,左心室射血分数70%,估测肺动脉收缩压102mmHg。右心房明显扩大,右心室壁增厚,运动正常,左心室内径大致正常,室间隔左移,左心室呈“D”字形,肺动脉扩张,腔内未见异常回声,三尖瓣瓣环扩张,致瓣叶对合欠佳,三尖瓣少~中量高速反流,余瓣膜结构、形态和启闭未见异常。超声诊断:重度肺动脉高压,右心扩大,三尖瓣中~大量反流。

肺功能:肺容量、肺通气功能正常,FEV1/FVC 95%,小气道功能正常,肺弥散功能明显减退。动脉血气分析:pH 7.46,PO2 67.7mmHg,PCO2 30.3mmHg,HCO3 -20.2mmol/L,SaO2 94.4%。

肺动脉增强CT(图10-3):主肺动脉及左右肺动脉明显扩张,未见狭窄及充盈缺损,右心房、室增大,双肺实质未见异常。诊断:肺动脉高压。

放射性核素肺灌注显像(图10-4):双肺非肺段性放射性稀疏,符合肺动脉高压改变。

右心导管检查:右心房平均压:10mmHg,肺动脉收缩压100mmHg,舒张压41mmHg,平均压64mmHg,肺毛细血管楔压:10mmHg,体动脉压:100/70/80 mmHg,全肺阻力:1319.2dyn?s/cm5,心指数:2.87L/(min?m2),吸入伊洛前列素(万他维)后急性肺血管反应性试验阴性。

肺动脉造影(图10-5):主肺动脉及左、右肺动脉明显扩张,外周纹理纤细,充盈不良,未见狭窄及充盈缺损。

【讨论】

2003年以前,临床上普遍采用原发性肺动脉高压来定义临床无明确病因的肺动脉高压。原发性肺动脉高压这个概念在1951年首次正式提出,在1965年,因食欲抑制剂阿米雷司投放欧洲市场而导致肺动脉高压流行后受到重视,进而研究发现减肥药相关的肺动脉高压在临床特点及病理改变方面均与原发性肺动脉高压相似,为此,WHO于1973年在瑞士日内瓦召开了第一次肺动脉高压专家工作组会议,总结了原发性肺动脉高压的临床和病理特点。1998年,法国Evian第二次WHO肺动脉高压专家工作组会议制定了肺动脉高压Evian分类标准,废除了继发性肺动脉高压这个比较模糊的诊断名称,原发性肺动脉高压被列为肺动脉高压类中的第一亚类,分为散发性和家族性两种类型。随着肺动脉高压研究的迅速进展,2003年威尼斯WHO第三次肺动脉高压工作组会议,对肺循环高压的诊断分类重新进行修订。由于原发性肺动脉高压原指病因未明的肺动脉高压,而目前已发现BMPR-2基因突变等重要病因,并且“原发性”往往与“继发性”相对应,容易造成混乱。因此,废除了原发性肺动脉高压这个诊断名称,改为特发性肺动脉高压。

特发性肺动脉高压发病率低,大约为(15~35)/100万。女性多见,女性与男性发病率之比为17∶1,诊断时平均年龄为37岁。在传统治疗时代,未经治疗的IPAH中位生存时间为2.8年,而1、3、5年生存率分别为68%、48%和34%。肺动脉高压的诊断标准为右心导管测定的肺动脉平均压超过25mmHg,肺毛细血管楔压、左心房压或左心室舒张末期压力低于15mmHg。

特发性肺动脉高压必须在除外各种引起肺动脉高压的病因后方可做出诊断,凡可引起肺动脉高压的疾病均应与特发性肺动脉高压相鉴别。本例患者为年轻女性,无家族史,通过血液化验、自身抗体检查、肺功能、肺动脉增强CT以及核素肺灌注检查,排除了结缔组织疾病、先心病、左心疾病、血栓栓塞性疾病、慢性低氧性疾病、药物及毒素等原因,并进行了右心导管检查,最后诊断为特发性肺动脉高压。特发性肺动脉高压尽管发病率低,但治疗效果不佳,预后差,往往类似恶性肿瘤,因此临床诊断需要格外谨慎,不可以未经临床详细筛查就草率诊断特发性肺动脉高压,给患者及家属造成巨大的心理压力。

治疗上特发性肺动脉高压患者首先应进行急性肺血管反应试验来判断是否需要使用钙离子阻滞剂。阳性者定义为肺动脉平均压至少降低10mmHg,且绝对值低于40mmHg,不伴有心输出量减少。阳性患者可以使用钙离子阻滞剂,从小剂量开始,注意心率和血压,并密切随访,观察其安全性和疗效,如果存在右心衰竭或血流动力学不稳定的IPAH患者不考虑CCB治疗,也不应接受急性肺血管扩张试验。一般治疗措施包括饮食、运动、适当的疫苗接种及避孕,氧疗维持血氧饱和度高于90%。所有IPAH都接受华法林抗凝治疗。对于有症状的右心室容量负荷增加的患者,可使用利尿剂。2009年ACCF/AHA指南指出,急性血管扩张试验阴性低危患者,则推荐的一线靶向药物治疗为内皮素受体拮抗剂或5型磷酸二酯酶抑制剂。而高危的患者推荐的一线治疗应为持续静注前列环素(依前列醇或曲前列素)。依前列醇可改善IPAH患者的运动能力、血流动力学和生存率,是大多数危重患者的首选治疗。曲前列素可通过持续静注或皮下注射给药。伊洛前列素是一种前列环素类似物,通过可调气雾装置每日6次给药。若单个靶向药物初治疗效不佳时,则考虑联合靶向药物治疗。对于尽管采用了最佳治疗,病情仍然进展的患者,可以选择心肺移植治疗。
