目前对嗜酸性食管炎（EoE）个体食管上皮的免疫组化和电子显微镜研究主要集中在与嗜酸性粒细胞激活有关的内容上。并且，这些研究结果已经确定了EoE的许多重要特征，包括嗜酸性粒细胞脱颗粒，阔边帽样囊泡形成等。虽然这些发现表明激活了的嗜酸性粒细胞可以作为功能性节点发挥作用，但已经有大量证据提出上皮屏障功能的改变在EoE的病理生理学方面起到了一定的作用。然而却很少有研究对炎症过程中上皮结构的改变进行描述。基

基于这些发现，我们提出一个假设：EoE的食管上皮内包含了某些超微结构，可引起食管上皮屏障的功能的改变。我们的研究数据发现儿童活动性EoE患者中食管上皮基底层的细胞桥粒数量出现下降。另外，我们还呈现了一系列在EoE上皮中出现的其他超微结构特征。这一研究的目的是为了阐明活动性和非活动性EoE的超微结构，并对研究对象的食管上皮进行调控。

我们前瞻性地对接受诊断性上段内镜检查来评价EoE的病人进行登记，随后从远端食管获取粘膜组织进行活检，并进行常规组织学和光镜评价。我们对所登记的患者的临床特征进行分析，并将他们分为4组：正常组、胃食管反流病（GERD）组、非活动性EoE组和活动性EoE组。另外，我们还对其食管粘膜基底层和浅表上皮进行回顾分析，希望找出组织内异常的改变，以及对基底层上3~4个棘细胞层的上皮表面的细胞桥粒数进行量化处理。

本研究一共登记了29个儿科案例（2-18岁）。我们观察到：与非活动性EoE、GERD和正常组相比，活动性EoE个体中的细胞桥粒数量（DPC）出现显著的下降。就DPC而言，非活动性EoE组与GERD或正常组相比没有明显差异。其他超微结构特征包括上皮微裂，嗜酸性粒细胞迁移，脱颗粒及形成阔边帽现象等。

与临床与分子水平的结果相一致，我们针对超微结构所获得的数据为儿童活动性EoE食管屏障发生改变这一设想提供了理论支持。这一发现有可能会提高后续治疗的效率。
