Blood:预测接受强化化疗的AML老年患者长期生存预后的新模型!

ALFA决策工具是一种简单、稳定且具有判别力的预后模型,可预测接受强化化疗的 60 岁以上的 AML 患者的总生存期

中心点| 7个基因的突变可独立预测≥60 岁且接受强化治疗的不同细胞遗传学组的 AML 患者的总生存期。

急性髓系白血病 (AML) 多见于≥60 岁的老年人。近年来,AML 患者的生存预后的改善仅限于 AML 年轻患者。无论是否进行同种异体干细胞移植(HSCT),强化化疗仍是所有 AML 成人患者(包括年龄较大、身体健康的患者)的标准疗法。

为了设计一个简单且可重复的分类器来预测 ≥60 岁的 AML 患者接受 7+3 方案治疗的总生存期 (OS),Itzykson等研究人员对来自 ALFA1200 试验的 471 名患者(中位 68 岁)的 37 个基因进行了测序。

84 名细胞遗传学风险不良的患者与其他 387 名细胞遗传学风险良好 (n = 13)、中等 (n=339) 或未检测 (n=35) 的患者的突变模式和 OS 均存在明显差异。TP53 和 KRAS 基因突变与细胞遗传学风险不良的患者 OS 更短独立相关(风险比[HR]分别是2.49,P=0.0003 和 3.60,P=0.001)。在无不良细胞遗传学风险的患者中,NPM1(HR 1.85, P=0.0005)或高等位基因频率(HR 3.51, P<10-4)、DNMT3A(HR 1.86,P<10-4)、NRAS (HR 1.54; P=0.019) 和 ASXL1 (HR 1.89, P=0.0003) 突变都可独立预测 OS。

结合细胞遗传学风险和这 7 个基因的突变,39.1%的患者可被分配到“go-go”组,2 年 OS 为 66.1%;7.6%的被分到“no-go”组,2 年 OS 为 2.8%;3.3%的属于“slow-go”组,2年 OS 为39.1%(P<10-5)。

3个验证队列的根据ALFA决策工具分组的患者的总生存期

在 3 个独立的验证队列中,分别有 31.2%~37.7% 和 11.2% ~13.5% 的患者被分到“go-go”组和“no-go”组,3 个试验队列的不同分组患者之间的 OS 均存在显著差异(P<10-5)。

总而言之,ALFA 决策工具是一种简单、稳定且具有判别力的预后模型,适用于预测接受强化化疗的 60 岁以上的 AML 患者的总生存期。
