CLIN CANCER RES近期发表了一篇文章,研究结直肠癌切除后循环肿瘤DNA检测是否可以鉴别高复发风险的患者以及循环肿瘤DNA检测是否可以早起发现肿瘤复发以及反映治疗效果。

在3年的随访期内,作者分析循环肿瘤DNA遗传学标志并用来检测微小病灶的存在以及监测肿瘤负荷情况。研究共纳入45例患者和371例血浆标本。27例患者的连续性标本出现术后循环肿瘤DNA,所有的复发患者(n=14)术后均出现循环肿瘤DNA,而无复发患者未出现任何循环肿瘤DNA。循环肿瘤DNA检测到复发的时间与CT检查相比平均提前9.4个月。21例局限性疾病接受治疗的患者中,6例在术后3个月内检测到循环肿瘤DNA。3个月循环肿瘤DNA分析预测复发的能力在23例肝转移患者中得到验证。复发干预治疗导致的循环肿瘤DNA水平改变与肿瘤负荷改变一致性良好。

文章最后认为,术后循环肿瘤DNA检测为残存病灶提供了证据,有助于鉴别高复发风险的患者。连续检测有助于早期发现肿瘤复发及为治疗反应提供信息。这些发现有助于结直肠癌患者术后的管理
