本研究旨在利用定量磁敏感图(QSM)研究多发性硬化(MS)患者深部灰质磁敏感性并评价磁敏感性与临床残疾的相关性。

本研究纳入了600例MS患者和250年龄、性别匹配的健康志愿者,并行3T MRI检查测量磁敏感性。利用感兴趣区和基于体素方法评价深部灰质磁敏感性。评价组间QSM和MRI容积差异及其与临床结果的相关性。利用方差分析、多元线性回归和基于体素分析比较组间差异性,探究MRI和临床结果的相关性。

结果为,与对照组比较,MS患者丘脑磁敏感性减低(-7.5 ppb vs -1.1 ppb; P < .001)、基底节磁敏感性增高(62 ppb vs 54.8 ppb; P < .001)。丘脑磁敏感性减低与病程延长(β = -0.42; P = .002)、残疾级别越高(β = -0.64; P = .03)和再次进展 (β = -4.3; P = .009)具有相关性。苍白球磁敏感性增高与残疾级别增高具有相关性(β = 2; P = .03)。在基于体素方法校正每个患者颅脑体积后,丘脑磁敏感性月底、苍白球磁敏感性越高仍然与残疾级别越高具有相关性(P < .05)。

本研究表明,定量磁敏感图(QSM)表明深部灰质铁沉积与多发性硬化进展(残疾程度)具有相关性,且不受组织萎缩影响。
