简要介绍:

溶栓治疗通过溶解动脉或静脉血管中的新鲜血栓使血管再通,从而部分或完全恢复组织和器官的血流灌注,最大程度减轻组织和器官的损伤,达到减轻患者症状并改善患者预后的目的。

20世纪80年代早期证实,冠状动脉(冠脉)内血栓形成是急性心肌梗死(AMI)的主要原因,随后展开了一系列有关抗栓及溶栓治疗的大规模临床试验。

1988年ISIS-2(Second International Study of Infarct Survival)研究结果显示,联合阿司匹林及链激酶溶栓可使AMI患者35 d病死率由13.2%下降到8.0%,因而确立溶栓治疗在AMI治疗的核心地位,标志着AMI进入“再灌注治疗”时代。

近年来经皮冠脉介入治疗(PCI)在心肌梗死急性期再灌注治疗中的应用得到长足发展。但溶栓治疗具有快速、简便、经济、易操作的特点,仍是目前急性ST段抬高型心肌梗死(STEMI)再灌注治疗的重要手段。

溶栓药物及分类

血栓的主要成分之一是纤维蛋白,溶栓药物能够直接或间接激活纤维蛋白溶解酶原变成纤维蛋白溶解酶(纤溶酶),纤溶酶能够降解纤维蛋白(原),促进血栓的裂解并达到开通血管的目的。根据发现时间的先后和药物的特点,可将溶栓制剂分为三代。

根据溶栓药物是否选择性作用于纤维蛋白(原)而分为非特异性纤溶酶原激活剂和特异性纤溶酶原激活剂两大类。临床应用时,建议优先选用特异性纤溶酶原激活剂。

(一)第一代溶栓剂

1.尿激酶(urokinase,UK):目前临床上所使用的UK是由人肾细胞合成并可从尿液中提取的一种蛋白水解酶,现多采用基因工程技术合成,通过水解Arg560-Val561肽腱,将血液循环中的纤溶酶原激活为纤溶酶。UK不具有纤维蛋白特异性,可出现全身性纤溶激活状态,增加出血风险。UK无抗原性和过敏反应,价格低廉,但其血管开通率较低,临床应用有一定局限性。

2.链激酶(streptokinase,SK):SK是从B型溶血链球菌培养液中提取的一种非蛋白酶的外源性纤溶酶原激活剂,能够与纤溶酶原以1:1比例形成SK.纤溶酶原复合物,催化纤溶酶原转化为纤溶酶,促使纤维蛋白溶解。SK并不具有纤维蛋白特异性,它对血液循环中以及与血凝块结合的纤维蛋白(原)均起作用,可出现全身性纤溶激活状态带来出血风险增加。SK具有一定抗原性,健康人群中多数可检测出SK抗体,部分患者输注SK时可出现过敏反应。

(二)第二代溶栓剂

1.组织型纤溶酶原激活剂(tissue type plasminogenactivator,t-PA):t-PA最初是从人黑色素瘤细胞培养液中提取的,生理情况下,t-PA具较弱的纤溶酶原激活作用,与纤维蛋白结合后可致构形改变,使t-PA与纤溶酶原结合力增加600倍,因此t-PA具相对纤维蛋白特异性,溶栓的同时不引起全身纤溶激活状态。目前临床上应用的阿替普酶(recombinant human tissueplasminogen activator for injection,rt-PA)是用基因工程技术制备的重组t-PA。阿替普酶结构中保留有两个环饼状结构(K区),对纤维蛋白具有特异性亲和力,故可选择性地激活血凝块中的纤溶酶原,使阿替普酶具有较强的局部溶栓作用。阿替普酶无抗原性,但由于半衰期短,需要持续静脉给药。

2.单链尿激酶型纤溶酶原激活剂(single chain urokinasetype plasminogen activator,SCU-PA):SCU-PA亦称前尿激酶或尿激酶原。可由人尿或肾胚细胞培养液中提取,也可采用基因工程技术制备,称为重组SCU-PA。尿激酶原系由411个氨基酸组成的单链尿激酶型纤溶酶原激活剂,具有酶原和酶的双重性,它在血浆中是惰性的,当血栓栓塞前部产生纤维蛋白Y/E片段时,尿激酶原与Y/E片段的c末端结合,激活纤溶酶原诱导血栓溶解,同时激活的纤溶酶将血栓附近的尿激酶原转化为UK,使血栓上结合的及周围游离的纤溶酶原大量激活,致血栓迅速溶解,尿激酶原诱导的溶栓具有相对血栓专一性,SCU-PA无抗原性,过敏反应少见。

(三)第三代溶栓剂

1.替奈普酶(tenecteplase,TNK-tPA):TNK-tPA是t-PA的突变体,分子中3个位点Th103、Asn117和LyS296-His-Arg-Arg299分别被Asn、Glu和Ala-AJa-Ala-Ala代替。TNK-tPA血浆清除呈双相性,起初半衰期为20~24 min,终末半衰期为90~130 min,临床上可单次静脉推注给药。TNK-tPA对纤维蛋白特异性较t-PA强,对血凝块有较大的亲和力,拮抗纤溶酶原激活抑制剂-1(PAI-1)的能力也较t-PA强。

2.瑞替普酶(reteplase,r-PA):目前国内临床应用的瑞替普酶通用名为重组人组织型纤溶酶原激酶衍生物或注射用瑞替普酶,属第三代溶栓药物,血管开通率高,临床应用方便。
