物理技术治疗冠心病的实践指南

2002年以来,我国冠心病死亡率总体呈逐年增长趋势,其中农村地区冠心病死亡率上升速度较快。传统的治疗方法,如药物治疗、经皮冠状动脉介入治疗、冠状动脉旁路移植术等治疗方法,虽为冠心病治疗带来较显著的疗效,但是使用传统方法治疗后,仍有多达14%的患者出现持续性心绞痛等症状。针对这些难治性心绞痛的患者还可使用准分子激光冠状动脉斑块消融术(Excimer Laser Coronary Atherectomy, ELCA)、体外心脏震波(Cardiac Shock Wave Therapy, CSWT)、增强型体外反搏(Enhanced External Counter Pulsation, EECP)、脊髓电刺激(Spinal Cord Stimulation,SCS)等物理方法治疗。激光治疗冠心病,可用于传统治疗方法无法治愈的复杂病变患者,并且通常很少出现并发症。体外心脏震波是治疗冠心病的新方法,可缓解冠心病难治性心绞痛,该技术在德国、意大利等国家已应用于临床。增强型体外反搏能够加快血流速度,作用于血管内壁,治疗患有稳定性心绞痛等冠心病患者。脊髓电刺激能够增加心绞痛患者的心肌供血,缓解患者心绞痛症状,应用于国内外的临床治疗。 激光和体外心脏震波等物理技术,能够有效治疗具有复杂病变的冠心病和难治性心绞痛。本指南旨在为使用激光、体外心脏震波技术、增强型体外反搏、脊髓神经刺激等物理技术治疗冠心病提供指导和方向。本指南主要分为四个部分,包括:准分子激光冠状动脉斑块消融术治疗冠心病、体外心脏震波治疗冠心病、增强型体外反搏治疗冠心病和脊髓神经刺激治疗冠心病。
