评估锻炼对低创伤总体和主要骨质疏松性骨折(髋部、脊柱、前臂或肱骨骨折)的影响,并确定锻炼计划监督的相应效果。

目的:评估锻炼对低创伤总体和主要骨质疏松性骨折(髋部、脊柱、前臂或肱骨骨折)的影响,并确定锻炼计划监督的相应效果。

方法:我们从2013年1月1日(即我们最后一次搜索的日期)到2021年5月22日,根据PRISMA指南对六个文献数据库进行了系统搜索,包括对照临床运动试验,这些试验包括(I)年龄≥45岁的个体,(ii)未接受骨折相关治疗/疾病的队列,(iii)观察期≥3个月,以及(iv)分别列出运动组(EG)和对照组(CG)的低创伤骨折数量。

结果:我们纳入了20项干预研究,包括21个试验组和20个对照组,试验组和对照组的参与年数分别为11.836年和11.275年。混合效应条件泊松回归揭示了运动对低创伤总发病率(率)比率(IR 0.67,95%可信区间[95% CI]0.51–0.87)和主要骨质疏松性骨折IR (0.69,95% CI 0.52–0.92)的显著影响。试验间的异质性对于低创伤总体来说是中等的(I2 = 40%),对于严重的骨质疏松性骨折来说是可以忽略的(I2 < 1%)。锻炼计划的监督在减少总体和主要骨质疏松性骨折中起着重要作用,其IR大约是主要监督组的两倍(IR 0.4495%可信区间为0.27–0.73和0.38;0.19–0.76)相对于主要无监督的运动试验(IR 0.8395%可信区间0.60–1.14和0.82;0.64–1.05).

结论:本研究为运动对中老年人低创伤性骨折和严重骨质疏松性骨折的积极影响提供了证据。锻炼计划的监督是骨折复位锻炼计划的一个重要方面。因此,基于家庭的锻炼协议应该越来越多地实施在线课程,以确保锻炼计划的广泛一致的监督和监控。
