感知膀胱充盈看起来简单,肾脏将废物和多余的水分送到膀胱,直到达到它的充盈阈值,膀胱就告诉中枢神经系统:该排尿了。该研究小组领导者MarkNelson博士发现,除了充盈压力,过程中还有被称为"非排尿瞬时收缩(transientcontractions,TCs)"的膀胱平滑肌收缩。膀胱平滑肌的瞬时收缩是充盈时传入神经活动的推力。这项研究刊登在4月发行的JournalofGeneralPhysiology杂志中。

TCs在感知压力和传递信号给感觉神经末梢的过程中起主导作用。TCs不仅给出膀胱何时充盈的信息,还提醒我们什么时候是排尿的最佳情形。所以研究人员得出结论,TCs是治疗膀胱功能障碍的新靶点。"TCs的有无、收缩发生的速度是导致膀胱活动过多或活动不足的原因。"Nelson博士说。

Nelson与同事们利用小鼠模型,探索了充盈压力、TCs诱导的瞬时压力对感觉神经刺激的相对贡献。他们发现,有固定的压力增长时,TCs比充盈压力诱导的感觉神经活动强10倍。研究人员认为,TCs在正常膀胱压力情况下的膀胱感知中起到主要份额。

尽管充盈压力并不影响TCs频率,但会影响达到最大承受压力的速度。后者的特点反应膀胱迫肌平滑肌的长度-张力关系的变化,而这是决定肌肉收缩效率的重要生理属性。这就意味着,TCs上升速率不仅告诉大脑膀胱的充盈情况,还告诉大脑,膀胱肌肉是否充分收缩以正常排尿。另外,他们发现,不管抑制小还是大电导钙激活的钾通道(SK和BK)--两者都对帮助放松平滑肌起重要作用--都能增加TC振幅和感官神经活动。

下一步,研究人员指出,是找到决定TCs提高的频率和速度的机制。"瞬时收缩在不同膀胱中表现不同",Nelson博士说,"至少在我们的实验中,频率是为某只动物或某个人特设的。看起来它们可以微调以得到最优反应"。
